All active DMAT team members must have the following immunizations to be deployable:

             TYPE             LAST DOSE

              DT                    7 years
              MMR                2 Doses
              PPD                 1 Year
              Polio                 1 Oral Series and 1 Adult Injection
              Chicken Pox     (disease or vaccination)
               Hepatitis A       2 shot series
              Hepatitis B       3 shot series
              Influenza           Yearly

It is the responsibility of each team member to ensure the immunizations listed above are current.

NOTE: Depending on the location of the deployment other vaccinations may be required before deployment.
