Policy:

Prior to Field Training and/or Deployment, each member must have on file in the Texas-4 DMAT office the following:

 1. History and Physical Exam
   >Current upon presentation and renewed every 2 years, except as may be indicated below; must include all medications.

2. Vaccinations (Required) 
    >Td
    >MMR
    >PPD
    >Chickenpox (disease or vaccination)
    >Hepatitis B (3 shot series)

3. Any of the following require a filed, written release from the member's treating physician:
    >Surgical Procedures (since last H & P)
    >New Onset Disease (since last H & P)
    >Workmen's Compensation Status
    >Pregnancy (all terms)

Specialized teams may be developed within Texas-4 DMAT which may require additional criteria for
participation.

Prior to Field Training/Deployment each member must complete a current medical status form to include current medications and an emergency contact number.

The Senior Medical Officer maintains the final decision on a member's medical readiness and may require an updated History and Physical prior to Field training/Deployment if at any time the health and safety of a member or team is in question.
